I really can't see the grooves in those pictures well enough to guess what caused them. It all just looks like a blurry mess. For all i know, i could be looking at a tree branch with splines carved into it lol.
 
lol i hear ya but thats as best as i could do, look at the third pic on the right it shows it the best, when i torque it down with the spacer it will make a grinding noise when i turn the axle so i have to loosen the nuts off to keep it turning smooth but when i do that the nuts arnt tight enough so there always coming loose after every ride .
 
Check out the break down of the rear axle in the OEM part of bike bandit...make sure you have all those parts.
 
Do you have a micrometer. Check how much it's off compared to your bearings. It sounds like your axle is worn.
