Chain jumping under full throttle

I've just had my 03 Warrior for a couple weeks. During last night's ride, accelerating very hard made a terrible racket. I looked underneath while riding and when I would accelerate even slightly aggressively, the chain would jump and buck like crazy. The chain tension seems good (although the sprockets look pretty worn). Can anybody help identify the problem? Could I be jumping teeth or does it sound like a transmission issue (and the chain is just a symptom)?

Excellent. Thanks. I just ordered the chain and associated parts. I wanted to make sure that wasn't a sign of real transmission problems first.
