How do I fix my loose rear brake disc?

The only thing I can think of is that the splines are worn between the disc and the axle. Anyone else had this type of wear issue? The disc is has play both forward and backward and left to right. And yes, the 4 allen bolts are tight.

the brake rotor is held in tight by the rear axle nuts. Check your rear bearings...mind was the same way and when i replaced the bearings everything tighten up good.
 
Ive seen forward and back but left to right is no good. Ive seen people wield the mount in place to the axle. Pretty common actually.
 
mine was getting giggily i cranked down the axle nuts a little and it shored up.. new bearings are in my future.
 
I think I'll put the big wrench to the nuts and see what happens next chance I get.
 
i used a massive pipe wrench.. allows for great grip and leverage
