clutch slipping, tried everything. please help!

Im new here so sorry If this has been posted already. I have been through the tutorials and problems section and tried everything. Ok, my clutch slips when I am in 3rd and 4th gear I have replaced the clutch cable with a new one, I also filed down the grooves in by clutch basket, I even added a steel plate in the pack. my clutch disks are in ok condition. when I am riding and it starts slipping if I just hold in the clutch lever a little bit then its ok, but I have tried adjusting the clutch on both ends and I still cant get it right. The only thing I have not tried is replacing the the clutch lever and perch, I dont know if that would make a difference, maybe if I get one of the adjust on the fly style perches? Also Im not sure if I have the cable adjusted right on the motor end of it, does anyone know how to adjust that? any help or any ideas would be greatly appreciated.
